Is that recent? I've seen pictures where the spline drive to the underspeed governor sheared on takeoff resulting in about the same.

yes was recent.
Both engines were operating normally.. it was merely pilot error.. why he tried to continue the TO when he departed the left side of the rwy is only known to him...
As the colonel says.. "it is a learning experience"
