Disturbing case of subtle incapacitation that the FO didn't fully recognize, or wasn't able to declare.

Sounds like the Captain had a complete mental breakdown.
She was trying but was ignored. She ended up in a situation far beyond her ability to handle.
This is where cultural values prevent any further action being taken. You cannot overcome thousands of years of culture with a few CRM courses.
ATC didn't help by repeatedly mixing up 02 and 20 - just added to the confusion imho. Why not just give a heading?
The Lionair CVR should be interesting when it is published.

You don't know the different types of incapacitation. Even though this case seems to be far from subtle, anything less than a pilot slumped over the controls in obvious medical distress is classified as "subtle".
It read like the FO was sharp and was doing her best to prompt the captain into action, but a combination of being a low-time trainee and possibly cultural factors kept her from ultimately taking control.
Two crew has prevented this in the past, just look at the AC incident where the FO had a breakdown and they ended up in Shannon.

With all due respect, this wasn't a two-crew op. It was a
(very bad) single pilot op. The pilot in the right seat ought
to be have been logging dual flight instruction received.

going bananas, and the FO having less than 400TT?
