Looks like Colonel Sanders was correct - too low/too slow and correct top gate not achieved.

Interesting to note that the Pilot hadn't been trained how to escape from this maneuver - that's a bit strange. Surely that is part of aerobatic training?

Not trained to escape from a loop?  Sombody's telling porkies.  How about a half-roll and exit via a Cuban 8?

There were things he could have done to salvage the situation, but I think that a brain fart was involved... 

Commencing the manoeuvre from lower than authorized...  Not a problem if you recognize it and it is deliberate.

Not using full power...  A puzzler and not easy to explain unless brainfart. 

Not realizing insufficient altitude gained...  A puzzler and not easy to explain unless brainfart.

Not rolling out when faced with the obvious...  A puzzler and...  etc.

It's almost like he was along for the ride and not really participating in the manoeuvre... 

Brainfart?  Hangover?  TIA? 

I have entered loops in a T-33 at 500 AGL and at 165 KIAS.  Successfully, obviously. Judicious use of flaps and power were key.  (Do not try this at home; I was young and single...) 

Given that the pilot survived, I wonder if he'll be subject to criminal/civil proceedings in light of those findings?

Having gone through the entire report my guess would be yes - 11 dead in an avoidable crash.

I'm not a lawyer or any kind of legal expert - but after reading through the report there seem to be some things that can be used in a civil suit at the very minimum.
